If you have a grass sickness survivor we could really do with your help! The Equine Grass Sickness Surveillance Scheme is conducting research into the issues experienced by survivors of chronic grass sickness. If your case was in the past five years we would be very grateful if you could fill in this survey and return it to the address on the form. Alternatively you can fill in an online version by visiting www.equinegrasssickness.co.uk.
